一.公司资质要求

1.开店主体公司登记注册文件，包括但不仅限于商业登记证/公司注册证，以及主体境内关联公司营业执照，并且开店主体需成立满2年及以上或其在境内提供相关服务的关联公司需成立满2年及以上；

1）公司登记注册文件中若含有有效期请确认文件在有效期内

2）公司登记注册文件中公司名信息请与开店主体公司中对应公司名一致

3）若公司信息存在变更（包括但不仅限于公司名变更/公司地址信息变更），请一并提交变更文件

4）非中/英文文件，需同时提交中文/英文翻译件，翻译件上需由开店主体公司的授权代表人签字

2.根据经营范围，需提交经营许可证或执业许可证等注册地法律法规要求的证照，其经营范围应包括对应科目，医疗服务需要在医疗机构的合法执业地点提供，如经营医疗及健康服务、疫苗服务一级类目，需提供符合医疗机构境外当地法律从事医疗服务相关的医疗机构资质证明文件以及其境内关联主体具备医疗健康服务经营范围的营业执照以及从事同类医疗健康服务所需的经营许可证或执业许可证；

3.授权代表人声明书，点此下载模板；

1）授权代表人声明书需要开店主体公司CEO或董事签字，并机打签字人姓名及职位

2）如被授权的代表人与签字人为同一人，需补充政府机构出具的董事证明或公司股权证明，香港公司可提供法团成立证明或年审文件

3）授权代表人声明书中有效期请填写具体日期信息（年/月/日形式），有效期截止时间不得早于当年自然年年底（续签季入驻的商家，有效期截止时间不得早于下一个自然年年底）

4.授权代表人声明书中被授权人身份证件；

3）此文件需有本人签字，签字名请与证件上名字信息一致

5.海外银账户开户证明或银行对账单；

2）商标注册类别需对应所要经营的商品（如：医疗服务类，商标注册类别需为44类）

3）商标注册证书需在有效期内

4）若商标存在变更（包括但不仅限于注册人变更/商标有效期延展/注册人地址信息变更），请一并提交商标局出具的变更证明文件

7.开店主体需为35类商标持有人或其关联公司，若开店主体为35类商标持有人关联公司，需提供商标持有人出具给开店主体公司的独占开店授权书，点此下载模板；

8.根据开店主体类型提供《合规经营承诺函》，点此下载模板；（只经营心理咨询类目，不需要该资质）

9.境内关联主体《无限连带责任保证函》，点此下载模板；（只经营心理咨询类目，不需要该资质）

10.经营体检、疫苗、医学美容类目需满足开店主体或其关联公司拥有全球范围5家以上合作的医疗机构；

二.品牌资质要求

1.需提供商标注册证书或商标注册申请受理函件原件扫描件，其中提供商标注册申请受理函件的需补充提供非R标情况说明。

2）商标注册类别需对应所要经营的商品（如：医疗服务类，商标注册类别需为44类）

3）商标注册证书需在有效期内

4）若商标存在变更（包括但不仅限于注册人变更/商标有效期延展/注册人地址信息变更），请一并提交商标局出具的变更证明文件

5）非中/英文文件，需同时提交中文/英文翻译件，翻译件上需由开店主体公司的授权代表人签字

2.开店主体公司非商标持有人时，需提交由商标持有人出具给开店主体公司的独占开店授权书：点此下载模板；

3.经营医疗及健康服务、疫苗服务一级类目的商家，还需提供如下资料：

（1）开店主体与下属或合作医疗机构之间的直接授权或合作协议；

（2）下属或合作医疗机构的符合境外当地法律从事医疗服务相关的医疗机构资质证明文件，其经营范围应包括对应科目；

（3）医疗服务需要在医疗机构的合法执业地点提供；

4.经营疫苗类目需提供：预防接种机构所在地符合法律法规要求的相应卫生管理部门的相关资质证明以及近3个月的疫苗进货单；

5.经营心理治疗/咨询、医疗服务、健康服务类目需提供：至少3个专业人员（心理咨询师、健康管理师、营养师、医师）执业资格证明及雇佣关系证明函；

1.需提供服务品牌发源地商标注册证书或商标注册申请受理函件原件扫描件或服务品牌发源地商标官网截图，其中提供商标注册申请受理函件的需补充提供非R标情况说明。

2.服务品牌商标持有人-开店主体一级授权；

1）授权书需商标权持有公司CEO或董事签字，并机打签字人姓名及职位（非CEO或董事签字，需补充提交授权签字函或公司内部决议以确认签字人有权签署相关文件）；如商标持有人为个人，需补充提供有本人签字的身份证件（护照/身份证/驾照）扫描件，证件需在有效期内

2）一级授权书中有效期请填写具体日期信息（年/月/日形式），有效期截止时间不得早于当年自然年年底（续签季入驻的商家，有效期截止时间不得早于下一个自然年年底）

3.经营医疗及健康服务、疫苗服务一级类目的商家，还需提供如下资料：

（1）开店主体与下属或合作医疗机构之间的直接授权或合作协议；

（2）下属或合作医疗机构的符合境外当地法律从事医疗服务相关的医疗机构资质证明文件，其经营范围应包括对应科目；

（3）医疗服务需要在医疗机构的合法执业地点提供；

4.经营疫苗类目需提供：预防接种机构所在地符合法律法规要求的相应卫生管理部门的相关资质证明以及近3个月的疫苗进货单；

5.经营心理治疗/咨询、医疗服务、健康服务类目需提供：至少3个专业人员（心理咨询师、健康管理师、营养师、医师）执业资格证明及雇佣关系证明函；

三.其他特殊要求

1.经营体检套餐、疫苗服务、基因检测、医学美容、医疗服务类目，商家入驻前平台会联合第三方对商家及医疗机构进行验商，需符合平台验商标准；

2.服务中使用的药品、试剂及医疗器械等均提供当地国/地区食药监管部门等相应法律法规要求部门的合法注册证明且三方机构确认合格；

3.医疗及健康服务和疫苗服务两个一级类目间可以互跨，禁止跨其他一级类目；医美、医学美容类目仅开放低危诊疗项。天猫国际对此保留最终解释权，并拥有不时对其更改的权利。

I. Company qualification requirements

1. Registration documents of the main company of the store include but are not limited to the business/institution registration certificate, certificate of incorporation, and business license of the affiliate located in mainland China. The main company of the store must have been established for at least 2 years, or its affiliate providing related services in mainland China have been established for at least 2 years.

(1) If any company registration document contains a validity period, ensure that the document is valid.

(2) The company name in the company registration documents must be consistent with the name of the main company of the store.

(3) If the company information (including but not limited to the company name and address) changes, a change document shall be submitted.

(4) Documents in languages other than Mandarin Chinese or English must be submitted along with a Chinese/English version. The translation must be signed by the authorized representative of the main company of the store.

2. The business license or practice certificate, and other licenses required by the laws and regulations of the place of registration, shall be submitted according to the business scope. The business scope shall include the corresponding services, and the medical services shall be provided in the legal place of practice of the medical institution. For example, if medical institutions based outside mainland China offer medical and health services, vaccination services, and other services under Category 1, the medical institutions shall provide the following materials: qualification documents certifying that the institutions are qualified to provide medical services in accordance with the applicable local laws outside mainland China, the business licenses of the affiliates located in mainland China and authorized with the business scope of medical and health services, as well as the business licenses or practice certificates required to engage in similar medical and health services.

3. The Authorized Representative's Statement shall be submitted. Click here to download the template.

(1) The Authorized Representative's Statement shall be signed by the CEO or Director of the Board of the main company of the store, together with the printed name and position of the signatory.

(2) If the authorized representative and the signatory are the same, a Director's certificate or the company's equity certificate issued by the government agency shall be provided. For Hong Kong companies, the corporate establishment certificate or annual review document may be provided as an alternative.

(3) In the Authorized Representative's Statement, the validity period shall be entered in the form of year/month/day, and the expiration time shall be later than the end of the current natural year. (For merchants who become registered during the renewal season, the end time of the validity period shall be later than the end of the next natural year.)

4. The identity documents of the authorized representative in the Authorized Representative's Statement shall be submitted.

(1) Identity documents include but are not limited to the ID card, passport, and driver's license.

(2) If any identity document contains a validity period, ensure that the document is valid.

(3) The identity documents shall be signed, and the signed names shall be consistent with the names on the documents.

5. The overseas bank account opening certificate or bank statement of the main company of the store shall be submitted.

(1) The overseas bank account opening certificate or bank statement must be issued by the bank (the name of which must be verified in the document). Documents issued by the company itself will not be recognized.

(2) The document shall clearly state the name and bank account information of the main company of the store.

(3) Documents in languages other than Mandarin Chinese or English must be submitted along with a Chinese/English version. The translation must be signed by the authorized representative of the main company of the store.

6. The Letter of Compliance Operation Guaranty shall be provided according to the type of the main company of the store. Click here to download the template. (This letter is not required for companies involved in only the business of psychological consultation.)

7. The Letter of Unlimited Joint and Several Liability Guaranty for the affiliate located in mainland China shall be provided. Click here to download the template. (This letter is not required for companies involved in only the business of psychological consultation.)

1. The business scope shall include the corresponding services, and the medical services shall be provided in the legal place of practice of the medical institution. For example, if medical institutions based outside mainland China offer medical and health services, vaccination services, and other services under Category 1, the medical institutions shall provide the following materials: qualification documents certifying that the institutions are qualified to provide medical services in accordance with the applicable local laws outside mainland China, the business licenses of the affiliates located in mainland China and authorized with the business scope of medical and health services, as well as the business licenses or practice certificates required to engage in similar medical and health services.

6. The original/scanned copy of the class-35 trademark registration certificate from the birthplace of the marketplace brand shall be submitted.

(1) A scanned copy of the original Class-35 Trademark Registration Certificate or acceptance letter of the Trademark Registration Application from the place of origin shall be provided for each brand in the marketplace. If the acceptance letter of the Trademark Registration Application is provided, additional information on the non-registered trademark is required.

(2) The trademark registration class shall correspond to the goods to be operated (for example, if the store operates medical services, the trademark registration class shall be 44).

(3) The trademark registration certificate shall be valid.

(4) If the trademark changes (including but not limited to the change of the registrant, extension of the validity period of the trademark, and change of the address of the registrant), a change certificate issued by the Trademark Office shall be submitted.

7. The main company of the store shall be a trademark holder of the trademark class-35 or an affiliate of the trademark holder. If the main company of the store is an affiliate of the trademark holder, the exclusive authorization letter issued by the trademark holder to the main company of the store shall be provided. Click here to download the template.

10. The main company of the store or its affiliates offering physical examination, vaccination, and medical cosmetology services must have more than 5 cooperative medical institutions worldwide.

II. Brand qualification requirements

1. A scanned copy of the original Trademark Registration Certificate or acceptance letter of the Trademark Registration Application shall be provided for each brand. If the acceptance letter of the Trademark Registration Application is provided, additional information on the non-registered trademark is required.

(2) The trademark registration class shall correspond to the goods to be operated (for example, if the store operates medical services, the trademark registration class shall be 44).

(3) The trademark registration certificate shall be valid.

(4) If the trademark changes (including but not limited to the change of the registrant, extension of the validity period of the trademark, and change of the address of the registrant), a change certificate issued by the Trademark Office shall be submitted.

(5) Documents in languages other than Mandarin Chinese or English must be submitted along with a Chinese/English version. The translation must be signed by the authorized representative of the main company of the store.

2. When the main company of the store is not the trademark holder, the exclusive authorization letter issued by the trademark holder to the main company of the store shall be provided. Click here to download the template.

3. Merchants operating medical and health services and vaccination services under Category 1 shall provide the following materials:

(1) Direct authorization or cooperation agreement between the main company of the store and its subordinates or cooperative medical institutions.

(2) The subordinates or cooperative medical institutions' qualification documents stating the business scopes and certifying that the subordinates or medical institutions are qualified to provide medical services in accordance with the applicable local laws outside mainland China. The services of the subordinates or cooperative medical institutions shall be included in the business scopes stated in the qualification documents.

(3) Medical services shall be provided in the legal place of practice of the medical institution.

4. For merchants operating services under the category of vaccination, relevant qualification certificates issued by corresponding health management departments in accordance with the laws and regulations of the place where the vaccination institution is located, as well as the vaccination purchase orders for the past three months shall be provided.

5. Merchants engaged in psychotherapy/consultation, medical services, and health services shall provide the professional qualification certificates and employment relationship certificates of at least 3 professionals (psychological counselor, health manager, nutritionist, or physician).

1. A scanned copy of the original Trademark Registration Certificate or acceptance letter of the Trademark Registration Application from the place of origin or a screenshot of the official website of the trademark in the place of origin shall be provided for each service brand. If the acceptance letter of the Trademark Registration Application is provided, additional information on the non-registered trademark is required.

2. The first-level authorization must be issued from the trademark holder of the brand to the main company of the store.

(1) The CEO or Director of the Board of the trademark holder shall sign the authorization letter, along with the printed name and position of the signatory (if the document is signed by a person other than the CEO or Director, an authorization letter for signing or internal decision letter of the company shall be submitted as evidence that the signatory has the right to sign relevant documents). If the trademark holder is an individual, a scanned copy of a valid identity document (passport/ID card/driver's license) signed by the individual shall be provided.

(2) In the first-level authorization letter, the validity period must be entered in the form of year/month/day, and the end time shall be later than the end of the current natural year. (For merchants who become registered during the renewal season, the end time of the validity period shall be later than the end of the next natural year).

3. Merchants operating medical and health services and vaccination services under Category 1 shall provide the following materials:

(1) Direct authorization or cooperation agreement between the main company of the store and its subordinates or cooperative medical institutions.

(2) The subordinates or cooperative medical institutions' qualification documents stating the business scopes and certifying that the subordinates or medical institutions are qualified to provide medical services in accordance with the applicable local laws outside mainland China. The services of the subordinates or cooperative medical institutions shall be included in the business scopes stated in the qualification documents.

(3) Medical services shall be provided in the legal place of practice of the medical institution.

4. For merchants operating services under the category of vaccination, relevant qualification certificates issued by corresponding health management departments in accordance with the laws and regulations of the place where the vaccination institution is located, as well as the vaccination purchase orders for the past three months shall be provided.

5. Merchants engaged in psychotherapy/consultation, medical services, and health services shall provide the professional qualification certificates and employment relationship certificates of at least 3 professionals (psychological counselor, health manager, nutritionist, or physician).

III. Additional special requirements

1. For merchants operating physical examination, vaccination services, genetic testing, medical cosmetology, and medical services, the Tmall Global platform works with a third party to carry out business inspection on the merchants and their medical institutions. The inspection results must meet the standards of the Tmall Global platform.

2. For the drugs, reagents, and medical devices used in the services, legal registration certificates issued by the food and drug administration department and other departments in accordance with the laws and regulations of the local country/region shall be provided, and shall be confirmed as qualified by a third party.

3. Cross-category operation is allowed between medical and health services and vaccination services in Category 1, but not allowed for the other services in Category 1. For the medical cosmetology category, only low-risk diagnosis and treatment services are allowed. Tmall Global reserves the right of final interpretation and occasional changes to the requirements.
